Employer Enrollment

Ready to get started? Our enrollment services are designed to make it quick and easy for you to establish your HSA program and get all your employees enrolled.

Enrollment Steps. Enrollments can work in a variety of manners but basically we need the following.
1 Complete the Employer Contribution Worksheet
Our Employer Contribution Worksheet [Excel version, PDF version] includes the following sheets for completion by the employer:
  • Employer Information Worksheet - the Employer Information Worksheet gathers basic information about the employer and asks questions about how you will run your HSA program.


  • Employee Contribution Worksheet - the Employee Contribution Worksheet collects information necessary to allocate the employer's contribution to the employees' HSAs. The columns collect information on the employee's names, initial contribution ($35 minimum) and whether or not the employer will be making monthly contributions. Please include a check for the Total Initial Funding amount with your forms (you can also transfer funds electronically, via ACH if you prefer). The annual custodial fee (a separate $25) will be deducted from the accounts during their second month and annually thereafter.

2 Have your Employees Complete Their Applications
Each employee needs to complete and sign an HSA Application. If the employer will be making contributions – employees can skip all of Section 2 on the HSA Application. Note: if your employees are transferring funds from another HSA custodian they need to complete and sign a Transfer Form in addition to their HSA Application.
3 Submit Completed Worksheet, Applications and Initial Funding
Congratulations you're done. Send your completed documents to:

  • Add/Remove Employees
    To add a new employee, simply add the employee’s name to the Employer Contribution Worksheet or to your own spreadsheet. To remove an employee simply strike the employee’s name off your spreadsheet and fax it to us at 320.323.4488 or e-mail us the name. Questions? Call 866.757.427


  • New Employees
    For new employees, add the employee as described above and have the employee complete and sign the HSA Application. Please include a check or authorize a one-time ACH debit for the set-up fee. We cannot open the account until it’s funded. If you prefer, just send the set-up fee along with your next payroll deferral and we will open and fund the HSA at that time.


  • Employer Consults
    HSA Resources will provide a consultation with the employer to discuss the HSA plan and related issues. Just click here to schedule an employer consult.

  • Enrollment Meeting Participation
    If you plan to have an enrollment meeting, HSA Resources provides a presentation for the employer or will lead the presentation via teleconference. HSA Resources will also provide an HSA expert to answer questions at an enrollment meeting via telephone. Or, just click to schedule an enrollment meeting.

  • Employee Education
    We also have a variety of tools available that you can download and distribute to your employees, some examples are shown below. We also have a site for your employees to research HSAs on their own leisure, Employee Resource Center.
  • HSA Overview Brochure                               Order copies
    This tri-fold brochure explains the basics of HSAs in an simple format. The brochure is intended for someone that may be aware of the concept but knows little else about HSAs.

    This tri-fold brochure explains the basics of the banking aspects of an HSA.
  • HSA Guide                                                        Order copies
    This multi-page guide explains HSAs in more detail. Employers can use it as a handout or as part of a presentation for employer group meetings.
  • Section 125 Plans
    A Flexible Spending Account, also referred to as a Section 125 Plan, is required if an employer wants to allow employee deferrals into an HSA on a pre-tax, pre-FICA/FUTA basis. We offer a combination HSA/FSA for this purpose. Please contact us if you would like to coordinate an FSA with your HSA.
